be completed by the applicant(s) or borrower with the Lender's or Broker's
assistance.
Therefore, if you don't know the answer
to a question just leave it blank and review with your Personal Loan
Professional
Most people ask about the first 2 check boxes at the top of the first page.

Check the 1st box if you are going to
use the income or assets of a coborrower.

Check the 2nd box if you are NOT going
to use the income or assets of a Spouse; the liabilities of a spouse must
be considered in many states. This is required because of Community
Property states or if property in another Community Property state is
being used as collateral.
If you have any questions please cover these with your Personal Loan
Professional. Depending on the loan you are applying for, you may not be
required to answer all the questions. For example,
if you are getting a Stated Income Home Loan you would not submit, tell or
disclose your actual income. You would leave this income blank but you would
give your employer's address, phone number and length of employment because
the lender/broker would verify this information.
